Application Wizard (Create a Program Application)
Create a program application using the Application Wizard. Log into your record, then:
- Accept eLogic Genesis' Terms and Conditions by scrolling to the bottom and mark the checkbox and tap Confirm
- Accept the agency's Release of Information (ROI) by scrolling to the bottom and mark the checkbox and tap Confirm
- The landing page will show all open applications at the bottom, tap to expand and review
- To start a new application select a program application from the drop down and tap Start Application
- You will see all the required information for the selected application
- Select a section
7. The wizard will walk through each question and then move to the next section
8. Progress can be tracked by viewing the top progress bar
Personal Information
This is where you will enter your basic information. Any information that is required by the agency will be outlined in red. All addresses in eLogic Genesis are subject to address verification.
1. Enter your contact information
2. When you are done, tap Save and Next
3. On the next page you will be required to enter your address, which is subject to address verification
4. Begin entering the address in the Adress 1 field box
5. Continue entering the address information into the required fields boxes
6. When you are finished, the Address Verification overlay will appear
7. If the address under Matching Address(es) is correct, tap on the address to select it and you have successfully verified the address
Note: In some cases, such as in rural areas or with new constructions, addresss may not to be verifiable. In such cases, it is acceptable to tap "use it as entered" in the Address Verification overlay. This will allow you to bypass the verification.
8. If the Matching Address(es) is not correct or there are no Matching Addresses, tap "Edit the address you entered"
9. The screen overlay will disappear, allowing you to make edits to the address fields
10. When you are finished making your edits, the Address Verification overlay will appear again
11. Once you have finished entering the information on the page, tap Save and Next
What if I have no address to verify?
We have hardcoded intake options for individuals who do not have an address to verify. Each of these options should be entered into the Address 1 field and will not work if they are spelled incorrectly. This will allow you to bypass the Address Verification.

Forms
If there is not a forms section, that means a form is not required for this application.
1. Tap Start under the required form
2. Fill out the form with the required information
3. Once completed, enter your name and date
4. Tap Finalize and you will be navigated to the next section
ID Proofs
If there is not an ID Proofs section, that means ID Proof is not required for this application. You will need to upload a file for each ID type required. Your caseworker will review all proofs of ID.
1. The dropdown menu will show you which types of identifications will fulfill the ID Proof requirements.
2. Select which type of ID you would like to upload
3. Click Upload and your file browser will appear
4. Find the file you'd like to upload and tap Open
5. When the upload is complete, you will see a green check mark, indicating that the ID Proof requirement has been fulfilled
6. If the wrong file has been uploaded, click the trashcan to delete
Note: To edit the file name, click on the pencil then edit the name. Click outside the field box to save the name.
Files
If there is not a Files section, that means a file is not required for this application.
1. Tap Upload below the required file type
2. Find the file you'd like to upload and tap Open
3. When the upload is complete, you will see a green check mark, indicating that the ID Proof requirement has been fulfilled
4. If the wrong file has been uploaded, click the trashcan to delete
Note: If there are additional files you'd like to share with the agency, click Upload Additional Files to upload additional files.
Budget
If there is not a Budget section, that means budgetary information is not required for this application.
1. The first question the Budget section will ask you is whether or not you have income, select Yes or No, depending on which applies to you

- If you select No, you will be navigated to the next section
- If you select Yes, the section will expand, allowing you to enter your budgetary information
2. Select your type of income from the drop down menu
3. Enter your income amount and frequency for that type of income
4. Tap Save
5. If you have addition income types, tap Add additional income source
6. Repeat steps 2 through 4 for each additional income source
7. When you are finished entering your budgetary information tap Save and Next
Family
If there is not a Family section, that means familial information is not required for this application. This is where you will enter information for your family members who live with you.
1. If you live alone, select the I Live Alone checkbox and tap Save and Next
2. If you do not live alone, select Add Family Member to expand the section
3. Enter the information for each family member who is living with you. When entering budgetary information, please refer to steps 1 through 7 in the Budget Section above
4. Tap Save Family Member
5. If you are done entering family members, skip to step 7
6. If you have additional family members, tap Add Family Member and repeat steps 3 and 4
7. When you are finished entring all household members, tap the No Additional Family Members check box
8. Click Save and Next
Assessments
If there is not an Assessments section, that means an assessment is not required for this application. This is where you will take any assessments that are required for the application. In some cases, you may have already done the required assessment recently. You will still be required to re-take the assessment to ensure it has the most up to date information.
1. Tap Start next to the Assessment
2. Tap on the applicable answer for the question and tap Save and Next
3. Repeat step 2 for each question in the Assessment
4. After you have completed the Assessment, you will be brought to the Review Screen
5. Review your answers

- You may edit an answer by tapping Edit
6. When you are satisified with all your answers, tap Finalize Assessment
7. A pop up will ask you to confirm the finalization
8. Tap Save and Next
Employment
If there is not an Employment section, that means employment information is not required for this application. This is where you will enter your employment information.
1. The first question the Budget section will ask you is whether or not you have income, select Yes or No, depending on which applies to you
2. Tap Save and Next
Submit Application
1. When you have completed each section of the application tap Submit Application
2. You will be presented with your application number
3. If you'd like to complete additional applications, tap Start New Application
4. To navigate back to your client record tap Exit Wizard
